FURTHER REMARKS:
1) Please, provide a copy of the Conference Call for Papers, together with this application form.
2) Supported Conferences must acknowledge financial support of the IEEE Control Systems Society.
3) Within 6 months of the conclusion of the conference, a report on the use of funding must be submitted to the Vice President of Member Activities.
