Please use the Microsoft Word version of this form if
at all possible - in preparing documentation for
merit and promotion cases, I'll need to convert
this information to Word format anyway and it
would save me a huge amount of extra work if it
was in that format already.
If you're up for a merit/promotion action and it's
been multiple years since you've turned in your biobib,
please combine all the years of your review period
into one biobib form if you can.
