Zeilinger Staff Excellence Award Nomination Form

Nominations must be received by 5 pm on Wednesday, May 8, 2013. . Nominees must be career staff in EECS or ERSO and must have provided a sustained level of performance for a minimum of three years. See Selection Criteria and Guidelines.

You will be sent a confirmation email for your nomination within a week of its submission. If you do not receive a confirmation email in 7 days time, please contact staff-award@eecs. You may wish to copy your letter as it is displayed on the confirmation web page and paste it into a file so you have a copy in case of any submission problems. You will NOT receive a copy of your nomination submission.
